School nurses are available during normal school hours to answer your healthcare questions and concerns. Our website will be updated regularly with resources for you and your family. This is required if your student needs an inhaler at school to be filled out by the doctor.
This form is for students who may need a special meal from the kitchen. Examples: Food allergies, Gluten, Dairy.
This form is to be filled out if you would like the school nurse and your pediatricians office to be able to communicate about your student.
If your student requires an Epi-Pen, it is a state law to have an Epi-pen action plan on file with the school nurse filled out by the prescribing doctor.
Medication Administration Form- Epi/ Albuterol permission
This form is for students who will need to take medication at school that is prescribed by a physician.
This form is for parents to fill out if you want the school nurse to give your child certain over the counter medications during school hours.

New Hampshire State Law RSA 200:32 mandates that a completed physical examination form be obtained prior to school entry. This physical exam must be performed within one year prior to the beginning of the school year. Please submit an up to date physical exam to the school health office.
